Non-muscle invasive bladder cancer (NMIBC) can be complex, but new treatments and strategies are helping patients maintain their quality of life while managing the disease. Understanding NMIBC
NMIBC includes tumors confined to the bladder’s surface or the first layer of the bladder wall, including flat lesions (carcinoma in situ or CIS) and papillary lesions. Even though these tumors are superficial, they carry a risk of progressing to muscle-invasive disease, making careful monitoring crucial.
Current and Emerging Treatments
Currently, all NMIBC treatment begins with transurethral resection of bladder tumor (TURBT). Next steps depend on risk level.
- Low-risk tumors: TURBT plus a single dose of intravesical chemotherapy, followed by regular cystoscopy.
- Intermediate-risk tumors: TURBT plus a year of intravesical chemotherapy.
- High-risk tumors: TURBT plus BCG therapy, sometimes up to three years. Select patients may also consider upfront cystectomy.
Emerging therapies focus on bladder preservation, including UGN-102 gel chemotherapy, TAR delivery systems, and immunotherapies like nadofaragene firadenovec. These innovations in NMBIC aim to improve treatment outcomes while reducing side effects.
BCG Therapy and Bladder Preservation
BCG remains a cornerstone for high-risk NMIBC. Researchers are exploring ways to enhance BCG efficacy, including combining it with immune checkpoint inhibitors and using modified strains. These bladder preservation strategies may help patients avoid surgery while still effectively targeting cancer cells.
